4. When placing your order, simply include these items:
   A. the number of calendar periods to display the ad
   B. the first calendar month the ad is to start
   C. method by which ad is being sent to Artsho
   D. the actual web page address to link it to
      (No web address? Then leave this blank)
   E. any new message to add to the web page
   F. additional comments to page programmer
   G. the available web site(s) home page.

5. Please do not pay by credit card within our

   emailed simultaneous to placing the order.
